The Delta State Government, in collaboration with organised labour, has constituted an Administrative Panel of Inquiry to investigate the circumstances surrounding the death of Mr. Adeda Ekwevugbe, a Deputy Director in the Ministry of Basic and Secondary Education.
The development followed concerns raised by colleagues and various stakeholders within the state public service after the death of the civil servant at his workplace.
In a special government announcement issued on Thursday, September 25, 2026, the Secretary to the State Government, Dr. Kingsley Eze Emu, said the Police had commenced its own investigation into the incident.
According to the government, consultations with organised labour resulted in an agreement to establish a separate administrative panel to examine the issues surrounding the death.
The eight-member panel comprises senior government officials and representatives of the major labour organisations in Delta State.
Mrs. Charity Ehimen, Permanent Secretary, Office of the Head of Service, will chair the panel.
